This subject will focus on a range of chronic conditions where the presentation and
management is often complex due to co-morbidities. Students will apply the principles
of chronic disease management to develop a rehabilitation project specific to the
context of their clinical practice.
Learning Outcomes
- develop a rehabilitation program specific to a community context and practice setting;
- demonstrate advanced skills to identify the rehabilitation needs of a community with
respect to chronic conditions;
- implement advanced strategies to identify current best practice for rehabilitation
of a range of chronic conditions;
- using advanced skills develop a risk management plan as it relates to chronic disease
rehabilitation;
- using advanced skills develop an evaluation plan for a chronic disease rehabilitation
program.
Subject Assessment
- Written > Proposal - (65%) - Individual
- Written > Self-reflection task - (35%) - Individual.
Special Assessment Requirements
Passing requirements: complete and submit all assessment items; achieve a cumulative
score of at least 50% inclusive of any penalties applied; attend professional development
activities.
